The Clinical Pharmacist will be responsible for providing comprehensive clinical pharmacy services focused on psychopharmacology and safe medication management for patients with acute and chronic psychiatric conditions. Review and validate medication orders, ensure appropriate dosing, monitor drug interactions, and provide therapeutic drug monitoring for psychotropic medications including antipsychotics, antidepressants, mood stabilizers, anxiolytics, stimulants, and medications used in addiction treatment such as opioid substitution therapies.

Collaborate closely with psychiatrists, nurses, psychologists, and multidisciplinary teams to optimize pharmacotherapy plans and enhance clinical outcomes. Participate in ward rounds, case discussions, and medication reconciliation processes to ensure safe transitions of care. Provide patient and family education on medication adherence, side effects, long-acting injectable therapies, metabolic monitoring, and risk management.

Monitor and report adverse drug reactions, support pharmacovigilance activities, and contribute to development and implementation of evidence-based medication protocols. Ensure compliance with Department of Health regulations, controlled drug policies, and hospital formulary guidelines. Oversee safe storage, dispensing, and documentation of narcotics and psychotropic medications in accordance with regulatory standards. Participate in quality improvement initiatives, medication audits, and accreditation processes. Support staff education on psychopharmacology updates, medication safety, and best practices in mental health pharmacotherapy. Maintain accurate clinical documentation and contribute to hospital medication management committees and governance frameworks.
