Responsibilities

• Conduct comprehensive health assessments to determine each patient's medical needs and care plan.

• Provide wound care and post-surgical nursing management for patients recovering at home.
• Educate patients and family members about self-care routines and healthy lifestyle choices.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care and optimize health outcomes.
• Maintain accurate, detailed, and timely patient records regarding health status and care given.
• Offer compassionate emotional support to patients and their families during challenging times.
• Assist with daily living activities like bathing, grooming, and meal preparation as needed.

Requirements

• Registered Nurse (RN) license with experience in home health care or community nursing.
• Strong clinical skills with the ability to manage diverse patient care scenarios confidently.
• Exceptional interpersonal and communication skills to interact with patients and families.
• Ability to work independently and make informed decisions in a home care environment.
• Reliable transportation and willingness to travel to patients' homes within the designated area.
• Physical stamina and capability to perform tasks like lifting, positioning, and transferring patients.
• Proficient in using electronic health records (EHR) systems for maintaining patient information.
