As a Home Care Nurse, you will play a critical role in providing personalized and compassionate care to patients in their homes. You will work closely with individuals and their families to manage various medical conditions, assist with recovery, and promote overall health and well-being. Your expertise will help patients achieve a higher quality of life and independence, all while maintaining a safe and supportive environment. You will be responsible for assessing patient needs, developing personalized care plans, administering medication, and collaborating with other healthcare professionals. This position requires an empathetic approach, attentive listening skills, and a commitment to enhancing the lives of our clients through quality healthcare services delivered at home.

Responsibilities

• Conduct comprehensive assessments of the patient's physical and emotional status.
• Develop and implement individualized care plans tailored to patient needs.
• Administer medications and treatments according to physician instructions and care plan.
• Monitor and record patient symptoms, vitals, and progress regularly.
• Educate patients and their families on healthcare management and disease prevention.
• Coordinate with physicians and other healthcare providers regarding patient care plans.
• Ensure all care provided is in accordance with medical and ethical standards.
• Provide wound care, intravenous therapy, and other acute care treatments as necessary.
• Support patients in daily activities to promote independence in home settings.
• Offer emotional support and guidance to both patients and their family members.
• Document care services and patient information accurately in the health records.
• Respond to emergencies and resolve patient issues promptly and efficiently.

Requirements

• Valid nursing license and registration as a Home Care Nurse to practice.
• Bachelor's degree in Nursing or a related healthcare field strongly preferred.
• Proven experience working as a nurse, specifically in home care environments.
• Strong understanding of medical terminology, principles, and practices.
• Excellent communication skills and the ability to work autonomously.
• CPR and First Aid certifications are required for this position.
