With a focus on patient-centered care, the Homecare Registered Nurse assesses patients health, develops tailored care plans, and works collaboratively with other healthcare providers to ensure comprehensive care. Responsibilities

• Conduct comprehensive assessments of patients' health conditions within their home settings.
• Develop individualized care plans tailored to the unique needs of each patient.
• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and record patients' vital signs and overall health status regularly.
• Coordinate with physicians and other healthcare professionals to adjust patient care plans.
• Provide educational support and guidance to patients and family members.
• Ensure a safe and sanitary environment for patients within their homes.
• Document all patient interactions, treatments, and changes in condition accurately.
• Assist patients with daily activities such as bathing, dressing, and mobility exercises.
• Promote healthy lifestyle habits and self-care strategies for optimal patient outcomes.
• Respond promptly to calls and emergency situations requiring immediate medical attention.
• Advocate for patients' needs and access to resources and healthcare services.

Requirements

• Current Registered Nurse (RN) license to practice .
• Bachelor of Science in Nursing (BSN) or associated degree from an accredited program.
• Minimum of two years nursing experience, preferably in homecare or similar settings.
• Demonstrated ability to work independently in various home environments.
• Strong communication skills to interact effectively with patients and healthcare teams.
• CPR certification and up-to-date knowledge of emergency medical procedures.
• Valid driver s license and reliable transportation for traveling to patients homes.
