Job Description

Carries out functions of selecting, procuring, storing, distribution, preparing, dispensing, administering, documenting and monitoring of pharmaceutical agents in the hospital.

Responsibilities

1. Dispensing medications to patients after checking and counseling.

1. Calling physicians to clarify a prescription when in doubt.

1. Prepare and /or oversees the preparation and dispensing of prescription medication to patients or medical practitioners. 

1. Suggests in generic substitution of prescribed pharmaceuticals. 

1. Oversees the procurement, acquisition, storage and disbursal of drugs and medications.

1. Adheres to storage protocols and maintains records as per requirements.

1. Reviews patient history and make assessments regarding probable reactions, allergies etc and communicate to concerned clinician.

1. Follow established departmental policies, framework, procedures and participate in QI progress.

1. Adhere to HAAD requirements and meet insurance documentation requirements.

1. Monitoring near expiry/expired medication and take necessary steps for disposal as per prescribed procedures.

1. Maintain confidentiality of patient information

1. Supports the formulary and restricted drugs program 

1. Supports the requirements of intravenous drug preparation program.

1. Checking upon prescriptions from all aspects before he/she dispense it.

1. Dispensing and register controlled and compounded prescriptions on their special books.

1. Keeping custody of controlled medication and abidance with its laws and regulations.

1. Continuously monitors and keeps track of all controlled medication, narcotics and antidotes. Maintain clear records of procurement, storage and disbursal/disposal of these.

1. Advises Physicians on equivalents or substitute drugs.

1. Helps reviews drug usage patterns and suggests control when needed.

1. Helps in frequent inspection of nursing drug cabinets, emergency trays and crash carts. 

1. Helps monitor pharmaceutical and other established indicators to ensure ongoing quality assurance.

1. Maintaining the antidotes levels, checking their expiry, adjusting the dose of different emergency medications and ensuring the standard storage of antidotes in Emergency department.

1. Maintaining the stock levels of different medicines in disaster trolleys.

1. Procurement, preparation, dispensing and storage of chemotherapeutic agents.

1. Abiding by the standardized practice of high alert medications and concentrated electrolytes.

1. Ensure compliance to all laws, circulars and regulations related to the pharmacy profession.

1. Review prescriptions to ensure accuracy and to ascertain and evaluate suitability. 

1. Assess and review possibilities of drug interactions and advise prescribing doctors accordingly. Also provide supporting input regarding side effects, dosage and proper medication use and storage

1. Check for drug-drug, drug-food/beverage, drug-disease interactions and update respective clinicians.
