Job Description
Roles & Responsibilities
Conduct psychosocial assessments and care planning
Provide counselling and psychosocial interventions
Case management, discharge planning & community liaison
Work closely with psychiatry, psychology & MDT teams
Desired Candidate Profile
BSc & Master s Degree in Social Work (Western Qualified, Tier 1 candidate)
Minimum 5 years post-qualification experience in complex mental health
Proven experience in leadership, supervision & MDT coordination
Strong skills in risk assessment, safeguarding & crisis intervention
Experience in addiction services preferred
Should qualify to be DOH licensed
