Job Description:
• Implement, develop and maintain an individualised EAP (Employment Assistance Plans) for supported employees in line with the person s career goals, and Intelife s Strategic and Operational Plans
• Responsible for the management of supported employees funding in accordance with the Company operational guidelines.
• Liaise with company Operations Coordinator to ensure compliance with all customer contracts regulations/policies and procedures.
• Liaise with families and other key stakeholder s to ensure the well-being of Intelife employees
• Ability to maintain accurate and timely records
• Participate in audits
• Identify training needs
Job Requirements :
• Knowledge of NDIS systems and processes.
• Understanding of person centered approach and individualized service provision.
• A current National Police Clearance (dated within the last three months or a willingness to obtain).
• Current WA driver s license
• Good analytical and problem solving skills.
• Excellent interpersonal and communication skills.
• A proactive and positive attitude to change.
• Knowledge of Microsoft Office suite of computer programs.
• Experience in working with people with significant disabilities (desirable).
• Knowledge of the ADE program and funding/reporting requirements (desirable)
• Understanding of the National Disability standards
