Roles & Responsibilities

Coordinate and manage patient care plans, ensuring timely and effective delivery of healthcare services.

Serve as the primary point of contact between patients, healthcare providers, and pharmacy staff.

Facilitate communication among multidisciplinary teams to optimize patient outcomes.

Monitor patient progress and follow up to address any concerns or changes in care needs.

Assist patients in navigating healthcare systems, including scheduling appointments and arranging referrals.

Maintain accurate and up-to-date patient records in compliance with regulatory standards.

Educate patients and their families about medication regimens, treatment plans, and available resources.

Identify and resolve barriers to care, advocating for patient needs and preferences.

Collaborate with insurance providers to verify coverage and obtain necessary authorizations.

Participate in quality improvement initiatives to enhance patient satisfaction and operational efficiency.

Desired Candidate Profile

  • Bachelor s degree in healthcare administration, nursing, pharmacy, or a related field.
  • Minimum of 3 years of experience in care coordination, patient advocacy, or a similar healthcare role.
  • Demonstrated ability to manage multiple cases and prioritize tasks effectively.
  • Excellent communication and interpersonal skills for interacting with patients and healthcare professionals.
  • Strong organizational skills with attention to detail and accuracy.
  • Proficiency in using electronic health records (EHR) and healthcare management systems.
  • Ability to work independently and collaboratively within a fast-paced environment.
  • Knowledge of healthcare regulations, privacy laws, and best practices.
  • Problem-solving skills to address patient needs and resolve issues efficiently.
  • Fluency in English; additional language skills are a plus.
