Job Description

Roles & Responsibilities

  • Conduct comprehensive assessments of clients' needs to develop tailored care plans that promote their well-being.
  • Coordinate services across multiple providers, ensuring seamless communication and continuity of care for clients.
  • Monitor client progress and adjust case plans as necessary to address changing needs and circumstances.
  • Facilitate access to community resources and services, empowering clients to navigate their support systems effectively.
  • Advocate for client rights and needs, ensuring their voice is heard in all aspects of their care and support.
  • Maintain detailed and accurate records of client interactions, services provided, and outcomes achieved.
  • Collaborate with multidisciplinary teams to create holistic solutions tailored to individual client challenges.
  • Provide crisis intervention and support, helping clients manage urgent situations and develop coping strategies.
  • Educate clients and their families about available resources, services, and rights within the healthcare system.
  • Participate in ongoing professional development to stay informed about best practices, policies, and regulations in case management.

Desired Candidate Profile

  • Bachelor's degree in social work, psychology, or a related field; advanced degrees are a plus for specialized roles.

  • Minimum of 4 years of relevant experience in case management or a similar client-focused role, demonstrating expertise.

  • Certification in case management (CCM, CMC, or similar) is highly desirable, showcasing commitment to professional standards.

  • Experience in healthcare, mental health, or social services sectors, equipping candidates with industry-specific knowledge.

  • Strong organizational skills, with the ability to manage multiple cases and prioritize tasks effectively under pressure.

  • Exceptional communication skills, both written and verbal, for clear interactions with clients, families, and service providers.

  • Proficiency in case management software and electronic health records (EHR) systems, enhancing operational efficiency.

  • Ability to work collaboratively in multidisciplinary teams, demonstrating a commitment to holistic client care and support.

  • Empathy and compassion, essential for building trust with clients and understanding their unique challenges and needs.
