Job Description
Roles & Responsibilities
Cooperate with other healthcare professionals as a member of the healthcare team to provide high-quality, coordinated, patient-centered care
- Review the medical record to determine the clinical status, the cause of admission and the clinical history of the patient
- Obtain and document a complete medication history to identify all of the patient s current medications, including regimens and administration routes, medication-taking behaviors, adherence, allergies, attitudes and experiences with medication therapy
- Perform daily rounds with primary physician and recommend the best interventions or modifications
- Analyze, document the implemented recommendations and evaluate the percentage of acceptance and rejection ratios
- Evaluate medication therapy and identify strategies to optimize medication therapy
- Identify medication-related problems, precautions, adverse side effects and evaluate collaboratively with other members of the healthcare team the need for intervention
- Cooperate with medication quality pharmacist in updating MDU guidelines and protocols
BSc. Degree in Clinical Pharmacy or Pharm D, American Board
Min. 2 Years experience
Working knowledge of healthcare quality, utilization review and related risk management processes application to various pharmacy committees and department/service activities.
Responsible for own professional growth beyond minimum preparation.
Must have integrity, initiative, evidence of good health and grooming, good moral character and right conduct, strong and pleasing personality
Participates actively in organization-wide activities without hesitance or personal reservation. Can adapt to unusual job-related circumstances.
Works effectively under stressful condition.
Willing to work extended hours to attend to emergency cases.
Willing to relocate in other SG Hospitals in order to fill in gap, if needed. Expert in dealing with clients
Desired Candidate Profile
