Job Description
Roles & Responsibilities
Provides in-depth pharmaceutical care to an adult or pediatric service by participating in patient care rounds with medical team, maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services within the integrated pharmacy model.
Assists medical staff with the selection of drug therapy and patient monitoring.
Reviews, verifies, and interprets medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for the patient s specific needs.
Assumes responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.
Conducts daily systematic medication reviews for patients on other floors, other than the floor covered by the clinical pharmacy consultant, whenever assigned.
Develops and evaluates therapeutic protocols/guidelines/pathways.
Provides real-time collaboration with nurses to create cost-effective medication care plans; focusing on medication misadventure prevention.
Detects, analyses, and reports adverse drug reactions as well as medication errors.
Acts as drug information and education resource for patients, students, colleagues, medical and nursing staff, and other healthcare professionals.
Documents therapeutic recommendations in the patient s medical record and pharmacy intervention forms.
Conducts Medication Utilization Evaluation (MUE) projects.
Leads teams for clinical projects and departmental initiatives, demonstrating cost saving research.
Follows all Hospital s related Policies and Procedures.
Participates in self and others' education, training and development, as applicable.
Desired Candidate Profile
Pharm.D./Bachelor s Degree in Pharmacy with accredited residency program (PGY1 or PGY2) is required.
Six (6) years of clinical experience after completion of accredited General Pharmacy Practice Residency (PGY1), or four (4) years of clinical experience after the completion of the Specialty Pharmacy Residency Program (PGY2) with Pharm.D./Bachelor s Degree is required.
Current Registration from country of origin and Saudi Commission for Health Specialties (SCHS) is required.
Board of Pharmacy Specialties (BPS) certification for all candidates is required.
A minimum of five publications in a peer reviewed journal indexed in PubMed or equivalent is preferred.
