Job Description

Roles & Responsibilities

  1. Conduct comprehensive geriatric assessments, including physical, cognitive, emotional, and functional evaluations

  2. Diagnose and manage acute and chronic medical conditions common in elderly patients

  3. Develop individualized care plans addressing medical, psychological, and social needs

  4. Monitor and manage polypharmacy and medication safety

  5. Coordinate care with multidisciplinary teams including nurses, physiotherapists, dietitians, and social workers

  6. Provide preventive care, health education, and lifestyle counseling to patients and caregivers

  7. Manage age-related conditions such as dementia, osteoporosis, falls, frailty, incontinence, and chronic pain

  8. Ensure accurate medical documentation and compliance with regulatory standards

  9. Participate in quality improvement initiatives and continuous medical education
