Roles & Responsibilities
- Conduct comprehensive health assessments to develop personalized care plans tailored to each client's unique needs and preferences.
- Administer medications and monitor patient vitals, ensuring adherence to prescribed treatment regimens and timely interventions.
- Provide wound care management and assist in post-operative recovery, utilizing advanced techniques to promote optimal healing.
- Educate clients and their families about health management, disease prevention, and lifestyle modifications to enhance quality of life.
Desired Candidate Profile
Registered Nurse (RN) with a valid nursing license and a minimum of GNM/BSC/ANM
At least 2 years of relevant experience in home healthcare or a similar setting, showcasing practical knowledge and skills.
Certification in Home Health Nursing or a related specialty to validate expertise and commitment to quality care.
Proficiency in electronic health record (EHR) systems and telehealth technologies to streamline patient management.
