The Home Care Nurse plays a critical role in providing personalized healthcare to patients in their own homes. As a Home Care Nurse, you will work with a diverse range of patients, including the elderly, individuals with chronic illnesses, and those recovering from surgery. Your primary goal will be to support and maintain the client s health and well-being by assisting with activities of daily living and providing appropriate care. This position requires a compassionate and empathetic approach, ensuring the patient feels comfortable and secure while receiving professional nursing care. Exceptional communication skills are vital for liaising with family members and other healthcare providers. The Home Care Nurse must be adaptable, resourceful, and capable of working independently to deliver high-quality healthcare services.

Responsibilities

• Assess patients conditions and consult with physicians to develop care plans.
• Administer medications accurately as prescribed and monitor effectiveness.
• Collaborate with multi-disciplinary team to deliver comprehensive patient care.
• Educate patients and their families on health care and self-care techniques.
• Monitor vital signs and record patient information and observations regularly.
• Assist patients with hygiene activities including bathing, grooming, and dressing.
• Ensure medical equipment is functioning properly and maintain supplies management.
• Provide wound care and change dressings when necessary to prevent infections.
• Facilitate patient rehabilitation exercises to enhance mobility and physical function.
• Document nursing interventions and patient responses to facilitate continuity of care.
• Coordinate with home healthcare aides and provide required supervision as needed.
• Respond promptly to any health or safety issues that arise at the patient s home.

Requirements

• A valid nursing license to practice as a registered nurse.
• Minimum of an associate's degree in nursing; a bachelor s is preferred.
• At least two years of nursing experience, preferably in home health care.
• Strong interpersonal skills with the ability to communicate effectively with patients.
• Capability to work independently and maintain high standards of patient care.
• Excellent organizational skills to manage multiple care assignments efficiently.
• Demonstrable emotional resilience and the ability to provide compassionate care.
