Job Description
Roles & Responsibilities
- Administer medications and monitor patient responses, ensuring adherence to prescribed treatment plans tailored to individual needs.
- Perform routine assessments of vital signs and overall health to detect any changes that require immediate attention.
- Develop personalized care plans in collaboration with patients, families, and healthcare providers to promote optimal health outcomes.
- Educate patients and caregivers on managing chronic conditions, medication usage, and lifestyle adjustments for improved quality of life.
Desired Candidate Profile
- Bachelor's degree in Nursing (BSN) or an Associate's degree in Nursing (ADN) from an accredited institution.
- Current Registered Nurse (RN) license in the state of practice, demonstrating compliance with local regulations.
- Minimum of 2 years of nursing experience, preferably in home care or community health settings.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is preferred.
