Duties and Responsibilities:

• Plan and prioritize visits to meet the health and scheduling needs of patients.
• Use the nursing process to develop, coordinate and implement care plans in collaboration with the patient/family.
• Complete thorough assessment, monitor and make clinical decisions for patients in their homes.
• Complete risk assessment of client s home to ensure safety of staff and patient.
• Advocate for the patient and family to ensure optimal outcomes
• Administer all types of medications safely and according to policies, procedures and home care competencies
• Proficient in operating all equipment required for patient care in the home.
• Works as part of a team of health care professionals.
• Works collaboratively with the multi-disciplinary health care team to ensure optimal patient outcomes.
• Documents according to home care policy.
• Acts in a professional and responsible manner.
• Provide health education and teaching to patient and family
• Participate in nursing continuing education program.
• Perform other tasks and duties within the scope of the knowledge, skills, and experience required for the job, as assigned by the immediate superior.
