Home Care Staff Nurse Key Responsibilities

1. Patient Care
   
• Provide direct nursing care to patients at home under physician guidance.
  
• Monitor vital signs, administer medications, and manage IV lines.
  
• Assist with wound care, catheter care, and other medical procedures.
  
2. Health Monitoring
   
• Observe and record patient conditions and progress.
  
• Report any changes in patient health to the supervising doctor or agency.
  
3. Medication Management
   
• Ensure timely and accurate medication administration.
  
• Educate patients and families on dosage, side effects, and schedules.
  
4. Patient Education
   
• Guide patients and caregivers on post-hospitalization care and recovery.
  
• Educate families about hygiene, nutrition, and disease prevention.
  
5. Documentation
   
• Maintain accurate nursing and medical records.
  
• Update daily reports and communicate with the care coordinator.
  
6. Supportive Care
   
• Provide emotional and psychological support to patients and families.
  
• Promote patient comfort and dignity during care.
  
7. Coordination
   
• Coordinate with physicians, physiotherapists, and other healthcare professionals for holistic patient care.
