Job Description

Roles & Responsibilities

  • Conduct comprehensive health assessments to create personalized care plans that meet each patient’s unique needs.
  • Administer medications and monitor patient responses, ensuring adherence to prescribed treatment plans.
  • Educate patients and families on disease management, medication usage, and health maintenance strategies.
  • Provide wound care and other therapeutic interventions, ensuring proper hygiene and infection control.

Desired Candidate Profile

  • Registered Nurse (RN) with a valid nursing license and at least an associate degree in nursing; a bachelor’s degree is preferred.
  • Minimum of 2 years of direct patient care experience in a homecare setting or similar healthcare environment.
  • Certification in CPR and Basic Life Support (BLS) to ensure readiness for emergency situations.
  • Experience with electronic health records (EHR) systems, ensuring accurate documentation and communication.
