Job Description

Roles & Responsibilities

  • Review and interpret prescription orders to ensure accuracy and appropriateness for patient needs.
  • Provide expert medication counseling to patients, addressing potential side effects and interactions tailored to individual health profiles.
  • Conduct thorough medication reviews, identifying and resolving drug therapy problems to optimize patient outcomes.
  • Collaborate with healthcare professionals to develop and implement comprehensive medication management plans.

Desired Candidate Profile

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution, demonstrating a strong foundation in pharmaceutical sciences.
  • Active pharmacist license in the relevant state or country, ensuring compliance with local regulatory requirements.
  • Minimum of 2 years of clinical pharmacy experience in a hospital or retail setting, showcasing real-world application of pharmaceutical knowledge.
  • Certification in Medication Therapy Management (MTM) or Board Certification in Pharmacy Specialties (BCPS) preferred, highlighting a commitment to specialized practice.
