Job Description

Roles & Responsibilities

1. Medication Experts

Provide knowledge on drug mechanisms, side effects, interactions, and dosing.

Help select the most appropriate drug therapy for individual patients.

2. Ensuring Safe & Effective Use of Medicines

Monitor prescriptions for errors, interactions, or contraindications.

Adjust doses for specific populations (children, elderly, pregnant women, renal/hepatic impairment).

3. Clinical Decision-Making

Participate in ward rounds and case discussions.

Recommend therapeutic alternatives or adjustments to improve patient outcomes.

4. Patient Education & Counseling

Educate patients on proper use of medications (how and when to take, side effects, storage).

Improve adherence to therapy, especially in chronic diseases.

5. Preventive Care & Public Health

Provide immunizations (in many countries pharmacists are certified vaccinators).

Participate in health screenings (e.g., blood pressure, blood glucose, cholesterol).

Promote lifestyle modifications and preventive health practices.

6. Medication Therapy Management (MTM)

Review all medications a patient is taking to ensure they are necessary, safe, and effective.

Identify and resolve drug-related problems.

7. Interprofessional Collaboration

Work with doctors, nurses, dietitians, and others to develop patient-centered care plans.

Ensure continuity of care during transitions (hospital discharge community care).

8. Pharmacovigilance & Research

Report and monitor adverse drug reactions (ADRs).

Participate in clinical trials, research, and development of new treatment guidelines.

9. Supply Chain & Medication Management

Ensure availability and quality of medicines in hospitals, clinics, and community pharmacies.

Manage formulary and contribute to cost-effective prescribing.

10. Specialized Roles

Clinical pharmacists in hospitals (ICU, oncology, infectious diseases).

Community pharmacists providing accessible care.

Industrial & regulatory pharmacists ensuring safety and efficacy of medicines.
