Job Description

Roles & Responsibilities

• Conduct comprehensive in-home patient assessments, including vital signs, physical examinations, and medication reviews, to establish a baseline and identify changing health conditions.
• Develop and implement individualized care plans tailored to each patient's specific needs and goals, ensuring alignment with physician orders and patient preferences.
• Administer medications, including oral, intravenous, and subcutaneous injections, and manage central lines, ports, and other specialized medical equipment.

Desired Candidate Profile

• Minimum of 2 years of experience in a clinical setting, preferably with experience in home health or a related field.
• Exhibits excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
• Must have the ability to work independently and make sound clinical judgments.
