Job Description
Roles & Responsibilities
- Conduct comprehensive in-home patient assessments, including vital signs, physical examinations, and medication reviews, to establish a baseline and identify changing health conditions.
- Develop and implement individualized care plans tailored to each patient's specific needs and goals, ensuring alignment with physician orders and patient preferences.
- Administer medications, including oral, intravenous, and subcutaneous injections, and manage central lines, ports, and other specialized medical equipment.
Desired Candidate Profile
- Minimum of 2 years of experience in a clinical setting, preferably with experience in home health or a related field.
- Exhibits excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
- Must have the ability to work independently and make sound clinical judgments.
