Roles & Responsibilities

  • Conduct comprehensive geriatric assessments, including physical, cognitive, and psychosocial evaluations, to identify complex health needs and functional limitations.
  • Develop and implement individualized care plans, collaborating with interdisciplinary teams to optimize patient outcomes and promote quality of life for older adults.
  • Manage chronic medical conditions common in older adults, such as cardiovascular disease, diabetes, and dementia, providing expert medical care and guidance.
  • Prescribe and monitor medications, considering age-related changes in drug metabolism and potential for adverse effects, ensuring medication safety and efficacy.
