Role Purpose

Provide treatment, care and support to babies, children, adults and elderly people with a range of conditions, including cleft palate, stammering, language delay, voice disorders and dysphagia (eating, drinking and swallowing difficulties).

KEY RESPONSIBILITIES

1. Offer Speech Therapy services by visiting patient home. .
2. Daily prepare medical bag to fulfil patient needs.
3. Therapist must reach his visit according to his schedule time.
4. Have responsibillity to keep the equipment and tools sterilized before and after each session.
5. Identify the speech and communication difficulty or disorder.
6. Assess the cause and nature of the problem, for example, congenital problems (such as cleft palate) or acquired disorders after a stroke or injury.
7. Devise and deliver a suitable treatment programme, working on a one-to-one basis to enable each of your patients to improve as much as possible.
8. Monitor and evaluate your patients progress.
9. Advise carers on implementing a treatment programme and train other professionals in therapy delivery.
10. Contribute to department s continuing medical education program.
11. Write confidential patient case notes and reports, as well as information for patients, carers and other professionals.
12. Paricipate in multidisciplinary team to devlop comprehensive and individualized care plan for each patients needs.
13. Fully aware and actively implement the departmental policies and procedures, local and international standards or requirements related to the department.
14. Participate in home care Safety Management & Risk Management Programs. Actively reports incidents in the system and whenever needed, participates in incident review.
15. Participate in patient and family education to help patients and families to better understand and participate in their care and make well informed care decisions.
16. Performs any other duties within the range of his/her professional knowledge and skills as directed by the direct manager.
