Patient Navigation and Support

• Acts as the primary point of contact for breast cancer patients throughout diagnosis, treatment, and follow-up.

• Educates patients and families on diagnoses, treatment plans, procedures, side effects, and follow-up requirements.

• Provides emotional support, counselling, and advocacy for patient needs.

• Assists patients with medical records and communication with referring physicians.

Clinical and Direct Patient Care

• Provides direct nursing care, including administering medications, monitoring vitals, and assisting in procedures.

• Conducts comprehensive patient assessments to identify functional, psychosocial, and healthcare needs.

Coordination of Multidisciplinary Care

• Facilitates communication and care planning among oncologists, surgeons, radiologists, radiation therapists, and other providers.

• Develops and implements individualized care plans with the multidisciplinary team (MDT).

• Allocates new patient cases to interventional radiologists and ensures appropriate tumour board presentation and follow-up actions.

Administrative and Clinical Management

• Oversees scheduling of diagnostics, biopsies, surgery, and follow-up appointments.

• Maintains accurate, up-to-date documentation in electronic medical records (EMR).

• Tracks internal and external referrals using tracker tools and monitors timely follow-ups.

• Prepares monthly reports to support quality improvement and KPI tracking.

Quality Assurance and Program Development

• Participates in audits, clinical trials, data collection, and continuous quality improvement projects.

• Contributes to development of patient education materials and clinical guidelines.

• Engages in professional development to stay current with best practices in breast care.

Communication and Liaison

• Acts as liaison among the hospital, external healthcare providers, and community services.

• Facilitates effective communication with physicians, nursing staff, patients, and their families.

• Advocates for patient-centered care and continuity across all stages of treatment.
