Duties & Responsibilities:

• Collaborate with healthcare staff to assess the significant social and emotional needs in accordance with the physical & mental condition of the patient.
• Develop policies & procedures for psychosocial assessment that defines factors facilitating/impeding healthcare progress, including, but not limited to; emotional, social, or psychological factors, home situation, financial factors, noncompliance to treatment & physical/mental disabilities.
• Comply with CBAHI standards, MOH regulations, and other relevant requirements.
• Maintain appropriate documentation of psychosocial screening and assessment results in the patient s medical record.
• Refer patient or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
• Establish policies and procedures regarding, but are not limited to, management of medico-legal cases such as alcohol, narcotic abuse, criminal acts, domestic violence & suicidal patients.
• Counsel patients individually or in group sessions to help them overcome dependencies & recover from illness.
• Provide social work services including short-term individual counseling, community resources planning and crises intervention.
• Maintain documentation of a patient s progress toward goals in accordance with professional standards of practices, policies, and procedures.
• Responsible for providing patient/family with appropriate community
  resources as needed.
• Make decisions based on individuals educational preparation regarding their health conditions & treatment.
• Maintain documentation of all relevant patient information in the medical record including, but not limited to; reason for referral, patient/family assessment, and reassessment results, care plan including goals and interventions such as counseling, education & facilitation of resources.
• Develop regular progress notes that include the patient/family understanding, care progress, and needs for different or additional services.
• Demonstrate and apply knowledge of health standards that ensure safety, quality of life, and protection of patient rights.
• Participate in discharge planning for patients requiring post-discharge services such as transitions to skilled nursing facilities, assisted living facilities, outpatient dialysis, acute rehab, hospice, and transportation needs.
• Provide age-specific education and information to patient/family, particularly related to coping, community resources, and advocacy.
• Monitor, evaluate, and record patient s progress according to measurable goals described in the treatment and care plan.

Qualification & Experience Requirements:

• Bachelor s Degree in social services or equivalent.
• Master s degree is preferred.
• Current and valid registration by SCFHS.
• Minimum 5 years of Social Services experience in a hospital setting.

Skills and Competencies:

• Knowledge of regulations established by the Saudi Commission for Health Specialties related to healthcare practices.
• Abilities to handle complaints, and disputes and resolve grievances or conflicts.
• Excellent verbal and written communication skills.
• Must have an in-depth understanding of many common medical conditions.
• Ability to explain medical concepts to patients in understandable language.
• Abilities to improvise operational efficiency, interdisciplinary collaboration, and medical records management.
